JOB TITLE: EPS Clinician
JOB PURPOSE:
The EPS Clinician provides clinical assessments and brief
treatment for high-risk families with multiple barriers to employment
and clinical consultation and in-service training to EPS program staff
in assigned regions and statewide as requested.
Masters in Social Work or a related field (clinical
license preferred) with a minimum of five (5) years of directly related
experience.
KNOWLEDGE,
SKILLS and ABILITIES:

Thorough understanding of home based service delivery
models

Knowledgeable of TFA/TANF and disability eligibility
guidelines

Familiar with factors and services impacting poor and
working poor families

Conversant with most recent DSM and able to use as a
diagnostic tool

Demonstrated ability in conducting home based clinical
assessments
-
Able to provide brief treatment
-
Experienced in child/adolescent, individual and family
therapy
-
Capable of leading in-service training sessions
-
Works well independently
-
Capable of maintaining a flexible schedule
-
Has access to a personal vehicle for region and
statewide travel
-
Dedicated to promoting cultural wellness on the job
-
Familiar with clinical services throughout designated
region and statewide as applicable to program services
-
Demonstrates basic computer literacy and ability to
work in a Windows environment

Demonstrated ability to work in a team approach
JOB
EXPECTATIONS:

Follows all EPS policies and procedures and represents
EPS program professionally within EPS program and in the community
-
Maintains a positive and respectful work attitude

Communicates proactively with EPS Regional Coordinator
and regional PSMs
-
Utilizes Efforts To Outcomes (ETO) to document all case
work in EPS program
-
Remains current with professional credentialing
requirements (if applicable) and best clinical practices
-
Actively participates as a member of the regional EPS
team
SPECIFIC DUTIES and RESPONSIBILITIES:

Reviews all initial client assessments with PSMs and
assigns/verifies clients’ level-of-service
·
Conducts home-visits, and in-home clinical assessments
and screenings of families identified as high-risk
·
Recommends long term treatment options and makes
appropriate referrals for client family members
·
Provides support and assistance to connect families with
services as necessary and appropriate
·
Provides in-home and/or community based brief treatment
services to focus on treatment readiness or brief treatment intervention
to stabilize families
·
Provides in-home short term clinical crisis intervention
services to families as needed while they await community based services
·
Provides clinical treatment and documentation for agency
billing of services as appropriate and directed by agency clinical
supervisor
·
Provides clinical consultation and training to EPS staff
as requested
·
Maintains accurate and timely documentation in the
Efforts To Outcomes (ETO) data management system
·
Attends regional and all staff meetings as requested
·
Actively participates in supervision with EPS Regional
Coordinator and agency clinical supervisor

Maintains a flexible work schedule to accommodate
program and client needs, including working non-traditional hours as
needed

Identifies and reports to EPS Regional Coordinator
trends, developments and patterns as they affect clients and service
delivery

Participates in agency and/or professional group
seminars/conferences to increase knowledge and accepts responsibility
for professional development.

Develops and maintains regular communication with key
regional community-based providers who provide treatment services
-
Facilitates client applications for exemptions and
disability as appropriate
-
Performs other reasonable duties as requested by EPS

Psychiatric Consultant: JFS is seeking a
Psychiatric Consultant for its outpatient mental health program.
Responsibilities will include psychiatric consultation and medication
management for approximately 2-3 hours per month at Jewish Family
Service of New Haven, Inc. Please contact Jonathan Garfinkle, Ph.D., JFS

Respite Child Care
Workers
4-19 hours per week to provide respite services to children.
Includes community, recreational and educational activities.
Bachelor in Human Services Required. Experience working with children
preferred Ability and desire to
attend periodic training sessions as well as CPR certification required. Valid driver's license and current
automobile insurance with reliable vehicle. Ability to document
visits and submit reports as necessary. $10-$15 per hour.
Salary is dependent on experience and education. We will match
respite workers with children and families referred through the
Department of Children and Families and area collaborative agencies.
